The safety of our patients and staff remains our top priority. At this time we will continue to operate in compliance with the Governor Mike DeWine orders and suggestions in regards to building occupancy restriction, social distancing and all other safety and health regulations.

Before your appointment:

  • If you are a new patient, or if your medical history needs to be updated, you will receive a separate email from our patient online portal. Please be sure to fill out your medical history online prior to your appointment.

  • You may now check in with the front desk and wait in our lobby prior to your appointment OR remain in your vehicle and check in by calling or texting the number below.

  • Everyone entering the office, both employees and patients, must wear a face covering at all times (i.e., mask, scarf, anything that covers your nose and mouth).

    Upon arrival, please remain in your vehicle and check in by texting or calling the front desk:

    You will soon receive a text or call from the office letting you know when your private, sanitized room is ready.

    Everyone entering the office, both employees and patients, must wear a face covering at all times (i.e., mask, scarf, anything that covers your nose and mouth).

    Screening
    Screening

    Everyone entering the building is screened for symptoms. You will be asked standard screening questions to assess COVID-19 exposure risk, and your temperature will be taken.

    If you do not feel well, have a cough or fever, shortness of breath, headache, change in sense of smell or taste, then please stay home and call your primary care physician.
